Selective reduction of genioglossal muscle activity by alcohol in normal human subjects.

What was done
Ventilation and genioglossal electromyographic (EMG) activity were measured in 12 awake, healthy subjects before and after oral ingestion of 1 ml of ethyl alcohol per kilogram of body weight. Recordings were conducted during quiet room air breathing and during hypercapnic rebreathing.
What was found
The abstract reports no numerical values, variances, or p-values. Alcohol ingestion did not alter minute ventilation, breathing patterns, or ventilatory responses to carbon dioxide. However, it significantly reduced genioglossal EMG activity during both quiet room air breathing and hypercapnic rebreathing, with a more consistent reduction observed in male compared to female participants.
Why it matters
This study provides a physiological explanation for why alcohol intake exacerbates obstructive sleep apnea, demonstrating that alcohol selectively impairs upper airway dilator muscle activity while leaving primary ventilatory pump drive intact.
Limits
The sample size was very small (n = 12), and exact numerical data, confidence intervals, and statistical tests were omitted from the abstract. The study lacked a randomized placebo-controlled comparator to account for time or task repetition effects, tested subjects while awake rather than during sleep, and was likely underpowered to evaluate sex-specific differences.
