What was done

Cones and rods subserving the central 43 degrees of vision were counted in 27 whole-mounted human retinas from donors aged 27 to 90 years with macroscopically normal fundi. Photoreceptor spatial density and topography were mapped and analyzed across age using specialized graphic and statistical methods.

What was found

Foveal cone density showed no consistent relationship with age or retinal location, and total foveal cone counts remained stable. In contrast, central rod density decreased by 30%, starting inferior to the fovea in midlife and forming an annulus of greatest loss at 0.5 to 3 mm eccentricity by the ninth decade. Vacated space was filled by enlarged rod inner segments, maintaining stable rod coverage across ages. At the temporal equator, cone density fell by 23%, whereas equatorial rod density remained stable across adulthood.

Why it matters

The findings demonstrate that aging selectively depletes central rods while sparing foveal cones, indicating that modest age-related declines in photopic visual acuity are not driven by foveal cone loss. They also reveal morphological compensation by surviving rods to preserve retinal coverage.

Limits

The study is cross-sectional and restricted to 27 post-mortem donor eyes, preventing longitudinal tracking of cell loss. Antemortem visual function, optical clarity, and subclinical retinal pathologies could not be evaluated directly from donor tissue.
