Gingival recession in relation to history of hard toothbrush use.

What was done
This cross-sectional study evaluated 182 adults aged 18 to 65 years with at least 18 natural teeth and no history of advanced periodontitis or periodontal surgery. Gingival recession was clinically recorded when the free gingival margin was apical to the cemento-enamel junction. Participants reported their history of hard toothbrush use (82 reported hard brush use, 77 reported no hard brush use, and 23 did not know).
What was found
Overall recession prevalence was 63%, increasing from 43% in younger subjects to 81% in older subjects (increasing by roughly 3.5 percentage points per decade). Females had approximately 4 percentage points fewer receded surfaces than males. Subjects with a history of hard toothbrush use had double the percentage of receded surfaces compared to non-users (mean 9.4% vs. 4.7%). Furthermore, higher brushing frequency correlated with a significant increase in receded surfaces among hard toothbrush users, but no such frequency-dependent increase occurred in non-users.
Why it matters
It highlights that toothbrush bristle stiffness, in combination with brushing frequency, may be an important behavioral contributor to gingival recession beyond age and sex.
Limits
The cross-sectional design precludes establishing causality. Exposure to hard toothbrushes relied entirely on retrospective self-report, introducing recall bias (demonstrated by 23 participants unable to recall their brush history). Brushing force, technique, toothpaste abrasiveness, and plaque scores were not reported.
