What was done

A randomized, double-blind, placebo-controlled crossover study investigated the acute effects of alcohol on plasma estradiol and estrone in an inpatient clinical research center. Participants were 12 healthy postmenopausal women on oral estrogen replacement therapy (1 mg/day estradiol with medroxyprogesterone acetate, tested during the estrogen-only phase) and 12 postmenopausal controls not using ERT. Each group consumed alcohol (0.7 g/kg) and an isocaloric placebo in randomized sequence on consecutive days.

What was found

Alcohol intake produced a 3-fold elevation in circulating estradiol in women on ERT, increasing from 297 to 973 pmol/L (81 to 265 pg/mL) within 50 minutes (P < .001) and persisting above baseline for 5 hours (P < .001). No significant estradiol change occurred in non-ERT controls. Estrone did not increase in either group, but decreased after both alcohol and placebo in the ERT group (P < .05). Peak blood alcohol levels (21 mmol/L at 50 to 60 minutes) did not differ between groups. Estradiol changes significantly correlated with blood alcohol concentrations on both the ascending (P < .001) and descending (P < .001) curves.

Why it matters

Moderate acute alcohol ingestion substantially increases circulating estradiol to levels 300% higher than targeted clinical levels in women using oral estrogen therapy, revealing a strong metabolic interaction.

Limits

The study had a small sample size (n = 12 per group) and only evaluated acute, single-dose effects over a 5-hour window. It examined only one oral formulation (1 mg/day estradiol), leaving transdermal delivery, other doses, chronic drinking patterns, and downstream clinical outcomes unmeasured.
