What was done

Researchers evaluated the relationship between crossed cerebellar diaschisis (CCD) and cortico-pontine cerebellar pathway damage in 29 patients with chronic supratentorial infarction. CCD was measured by side-to-side cerebellar count asymmetry on 123I-IMP single-photon emission computed tomography (SPECT). Neuronal damage was graded via computed tomography (CT) density scores (0 to 2) across four pathway regions: frontal association, motor and premotor, sensory and parietal associations, and the posterior limb of the internal capsule.

What was found

Two-factor factorial ANOVA showed a statistically significant difference in CCD rate based on the tissue density score (score 0 or 1 vs score 2, p < 0.05). Multiple stepwise regression demonstrated a significant correlation exclusively between CCD and necrotic density (score 2) in motor and premotor areas (r = 0.515, p < 0.05).

Why it matters

These findings suggest that damage to motor and premotor projections along the cortico-pontine cerebellar pathway is a primary contributor to functional cerebro-cerebellar disconnection in stroke.

Limits

The sample size is small (n = 29) and derived from a single center. The abstract does not report clinical or motor functional outcomes, and CT density scoring provides lower resolution than modern MRI techniques.
