Physiological Profile of Middle-Aged and Older Climbers Who Ascended Gasherbrum II, an 8035-m Himalayan Peak

What was done
Body composition, cardiac structure, and respiratory gas exchange during exercise were measured in eight middle-aged and older Japanese climbers (seven men, one woman, aged 54 to 63 years) 6 months prior to an expedition to Gasherbrum II (8035 m). The climbers had differing levels of extreme-altitude experience.
What was found
Using supplemental oxygen, the four experienced climbers reached beyond Camp 4 (7400 m) without health issues to attempt the summit, whereas the four climbers with minimal extreme-altitude experience developed altitude sickness on the way to Camp 4. Body fat percentage ranged from 14.9% to 21.4%. Peak VO2 ranged from 30.9 to 45.6 ml/kg/min and showed no significant association with ascent success. Three experienced male climbers had borderline hypertension and eccentric left ventricular hypertrophy, while cardiac structures were otherwise generally normal.
Why it matters
This study suggests that sexagenarians do not need elite-level aerobic fitness to climb 8000-m peaks with supplemental oxygen, highlighting the importance of experience and moderate fitness over extreme VO2 max.
Limits
The sample size was very small (n = 8) with only one female participant, limiting statistical power and generalizability. The study lacked a non-older control group and did not control for pacing, individual acclimatization rates, or in-field physiological measurements at altitude.
