The impact of social isolation and digital exclusion on mental and physical health in older adults: A meta-analysis.

What was done
Systematic review and meta-analysis of 11 primary studies (9 observational, 2 randomized controlled trials) published since 2000, including over 350,000 community-dwelling older adults from North America, Europe, and Asia. Random-effects models were used to calculate pooled odds ratios and 95% confidence intervals for depression and cognitive decline. Physical function outcomes were synthesized qualitatively.
What was found
For depression risk (analyzed in 111,784 participants), the pooled odds ratio was 1.60 (95% CI: 1.39-1.84, P < .001) with moderate heterogeneity (I2 = 43%), indicating a 60% higher likelihood of depressive symptoms among socially or digitally disconnected older adults. Cognitive decline (3 longitudinal cohorts) showed a non-significant pooled odds ratio of 1.03 (95% CI: 0.92-1.15; I2 = 55%). Physical functioning outcomes, including Short Physical Performance Battery scores and activities of daily living disability, were consistently worse among isolated or digitally excluded individuals. No small-study bias was detected.
Why it matters
This review links digital exclusion and social isolation to elevated depression risk and worse physical function in older adults. It suggests interventions addressing both digital literacy and social connectivity could benefit geriatric mental health.
Limits
The evidence base is predominantly observational (9 of 11 studies), limiting causal inference. Physical functioning outcomes could not be quantitatively pooled due to insufficient data, and cognitive decline analysis was limited to 3 cohorts. The abstract pools social isolation and digital exclusion together without reporting separate quantitative effect sizes for each.
