In a clinical trial led by Markus Bock, a single seven-day cycle of a fasting-mimicking diet followed by a Mediterranean diet produced better outcomes in multiple sclerosis patients than six months of a continuous ketogenic diet.

In the randomized pilot clinical trial led by Markus Bock and colleagues (NCT01538355, published in Cell Reports 2016), 60 relapsing-remitting multiple sclerosis patients were assigned to a control diet, a continuous ketogenic diet for 6 months, or a single 7-day cycle of a fasting-mimicking diet (FMD) followed by a Mediterranean diet for 6 months. The trial reported preliminary feasibility and quality-of-life improvements in both dietary intervention groups, but it did not find that a single cycle of FMD was superior to 6 months of a continuous ketogenic diet. Furthermore, a follow-up biomarker analysis of neuroaxonal damage from this exact cohort (PMID 34764215) demonstrated that 6 months of the ketogenic diet significantly reduced serum neurofilament light chain (sNfL) levels compared to control, whereas the single 7-day fasting cycle had no significant effect on sNfL.
